NOW HIRING: 1:1 Supportive Services/IM/PMI Lead Care Coordinator – Burtonsville/Baltimore, MD

Job Title: Lead Care Coordinator
Salary/Hourly Rate: $58,000 – $63,000 annually
Position Status: Full Time W-2
Location: Burtonsville/Baltimore, MD
Reports To: Operations Manager

Position Overview:

The Care Coordinator is responsible for the support of the success and operational excellence of multiple mentoring programs, including the 1:1 Supportive Services, Individual Mentoring, and Parent Mentoring Initiatives (PMI). This role requires strong leadership, high-level communication, and administrative oversight of referral and service delivery processes. The ideal candidate will build and maintain relationships with referral sources, work closely with the scheduler to manage incoming clients and appointments, and provide strategic support to mentor supervisors and mentors. This individual will serve as a key assistant ambassador for Lead4Life and a liaison between community partners, case managers, and internal staff.

Responsibilities (Including but not limited to):

  • Assist in the overseeing daily operations of mentoring programs (e.g., 1:1 Supportive Services, mentoring, Parent Mentoring Initiative (PMI), Individual Mentoring program)
  • Serve as a primary point of contact for communication with referral sources (ie. DSS case managers, schools, courts)
  • Coordinate closely with the scheduler to manage referrals, intakes, and client appointments, including doctor visits and support service scheduling.
  • Acting as liaison between the organization, mentors, participants, families, and external partners.
  • Maintain administrative access and oversight of all program documentation to ensure compliance with HIPAA, Federal, State, and grant requirements.
  • Work in close collaboration with and provide support to the Mentor Supervisor, Lead Scheduler and mentor team; act as a liaison to address challenges and manage crises in real time.
  • Assist in the onboarding process, including interviewing, screening, and onboarding of new mentors.
  • Provide behavioral support and guidance to the APY mentor team.
  • Ensure consistent communication and documentation across all mentoring programs—emphasizing accuracy, timeliness, and clarity.
  • Support the development and implementation of individualized service plans for all participants.
  • Report all incidents and allegations in compliance with CARF standards and organizational policy.
  • Build and sustain partnerships with stakeholders, hotels, and agencies to increase awareness and drive referrals.
  • Represent Lead4Life at community events, fundraisers, and committee meetings.
  • Provide timely reporting on program metrics (monthly, quarterly) and assist with grant support documentation in partnership with PMA.
  • Support ad hoc projects and emerging organizational needs as assigned.

Minimum Requirements:

  • Bachelor’s degree in human services, nonprofit management, social work, education, business, or a related field or equivalent work experience and a combination of education totaling at least five (5) years in a leadership or program management role will be considered.
  • Demonstrated experience working with both micro- and macro-level programming.
  • Proven ability to lead, mentor, and support high-performing, mission-driven teams.
  • Strong communication and interpersonal skills, with an emphasis on documentation, follow-through, and real-time problem solving.
  • Familiarity with behavioral support strategies and trauma-informed care practices.

Desired Qualifications:

  • Experience with strategic planning, performance measurement, organizational development, and program compliance.
  • Proficiency in Microsoft Office Suite and standard office technology; ability to work within HIPAA guidelines.
  • Collaborative and adaptable, with a growth mindset and creative problem-solving abilities.

General Physical Requirements:

This position primarily involves sedentary work, including prolonged periods of sitting and working at a computer. Occasional standing, walking, bending, or light physical activity may be required. The role may also involve periodically lifting or moving items weighing up to 50 pounds. Flexibility to adjust positions throughout the workday is necessary to meet the physical demands of various tasks.

Physical Activities:

The position requires general physical abilities including reaching, hand and finger dexterity for typing and handling materials, and the ability to grasp objects as needed for daily tasks. Effective verbal communication is essential for conveying and exchanging information clearly with colleagues and stakeholders. Adequate hearing is necessary to engage in conversations, respond to verbal cues, and interpret spoken information accurately in a variety of settings.

Visual Acuity:

The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; expansive reading; visual inspection involving small defects, small parts and/or operation of machines (including inspection); using measurement devices; and/or assembly of fabrication of parts at distances close to the eyes.

Working Conditions:

None: The worker is not substantially exposed to adverse environmental conditions (such as in typical office or administrative work).
