Navigation Services is a program that is designed to help youth and families who are at risk of homelessness, facing housing instability, or families who have been affected by incarceration. This program is intended to break the barriers that are contributing to housing instability, whether it is for educational purposes, unemployment, or family difficulties. This program is funded through the Wicomico Partnership for Families and Children, the Local Management Board for Wicomico County, and the Maryland Governor’s Office of Crime Prevention: Youth and Victims.
Participant Profile: Navigation Services’ participants must be between the ages of 16-24, or a family in need, with at least one child who is a minor.
Breakdown of Services: Lead4Life staff provide participants with a warm handoff of services located within the community, beginning with a thorough intake assessment to identify the participants’ barriers, which are hindering their success. Lead4Life staff then provides participants with a weekly check-in phone call or visit to educate them on pertinent programs/services and to ensure that participants are taking advantage of the services offered in the community. In addition to connecting participants to beneficial resources, Lead4Life’s staff also partners with local organizations for community service events such as the Fenix Youth Project’s food distribution and voter registration events, Adopt-a-Block’s food distribution event, an Opioid Awareness outreach event, and more.
Lead4Life’s staff has navigated participants to resources and services including, but not limited to: DSS, Joseph House, Mental Health Organizations, HOPE Ministries, Salvation Army, Local Care Team, Judy Center, Wicomico County Board of Education, and more.
