Position Overview:
The Rehabilitation Counselor (RC) is responsible for providing support to the Rehabilitation Specialist (RS) to assist individuals with severe and persistent mental illness, to develop or restore independent living and social skills, including the ability to make decisions regarding life, self-care, illness management, and community participation; and to promote access to and use of community resources to facilitate the individual’s integration into the community, thereby facilitating recovery, and preventing relapse and re-hospitalization.
Primary Function:
Support the PRP staff, participants, and program structure:
- Plans, manages and coordinates the PRP.
- In collaboration with the RS and PD, staff implements programs, goals, and plans and recommends broad organizational policies governing the operation of the Organization.
- Assures that the staff complies with relevant standards and regulations.
- The person filling this role must be able to educate direct staff on proper documentation, goals, and the IRP/DLA20.
- The PD takes responsibility in preparing the Organization for RS COMAR regulation changes that will impact operations.
- Assist with managing all PRP site locations.
- Travel Statewide to other PRP locations.
- Pursues opportunities for local, state, and federal funding for new or existing programs.
- Assist in the preparation and submission of budget proposals/projects to RS.
- Assures service quality standards.
- Submits expenditures as required by the Organization’s budget and financial authority policies to the Executive Director.
- The PD is responsible for the coordination of rehabilitation functioning with information services.
- Assist in data collection for qualitative and quantitative comparison of service needs with current services.
- Creates outreach strategies to ensure a revolving door with referrals.
- Builds relationships with key stakeholders in the program such as Local Behavioral
- Health Associations, therapists, hospitals, etc.
- Serves as a staff spokesperson for public and community relations.
- Attends general staff meetings and committee meetings as requested.
- Assist in supervising and evaluating employees.
- Assures that the staff complies with relevant standards and regulations.
- Responsible for, and actively participates in, public education and public relations within the service area.
- The PD helps ensure that participants have a recognized voice in Organization functioning.
- Assistance with CARF Accreditation preparation and ongoing compliance.
- Attend professional development opportunities.
Specific Responsibilities:
- Assist clients in gaining interpersonal skills.
- Thorough documentation identifying client symptoms or behaviors and interventions aimed at assisting client with needs.
- Participation in developing client interventions with treatment teams
- Provide client interventions in social skills training to improve communication skills and facilitate appropriate interpersonal behavior.
- Problem-solving, support, and supervision of clients related to program and social activities.
- Assist clients in gaining and utilizing skills necessary to participate in community reintegration– educational groups, behavior control, social skills, communication skills, ADLs support, successful interpersonal behavior, and coping skills.
- Provide interventions and services to promote the highest possible level of success.
- Provide strategies in designing planned interventions for complex clients by providing feedback to the treatment team.
- Collaboration with outside agencies to arrange for community services beneficial to clients, in addition to provision of ongoing collaboration between the treatment team.
- Have input into and assist care planning for clients including completion of paperwork prior to discharge, addressing aftercare needs and placement plans.
- Facilitate or co-facilitate groups arranged by the clinic or PRP programs.
- Complete progress notes for services delivered by the appropriate deadlines.
- Document all contact with other agencies and for all sessions with clients.
- Attend and contribute to treatment planning sessions.
Essential Function:
- Complete three (3) visits each month for minors and six (6) visits each month for adults.
- Visits are to be a combination of in-home/community and on-site/office to ensure clients receive both individual and group services.
- Complete and submit all documentation for client visits.
- Complete and submit monthly documentation the day before the last day of the month by COB.
- Complete, have all signatures, and submit Treatment Plans for clients every 90 days (review).
- Engage in supervision every month with a direct supervisor.
- Attend staff meetings and trainings every month and as scheduled.
- Collaborate with the client’s therapist and psychiatric provider every month for the continuation of goals developed for each client.
- Collaborate with other individuals pertinent to the client’s overall care (Parents, guardians, community providers, court representatives, teachers, etc.) to monitor behavior, address barriers to goals, needs to be addressed, etc. on at least a bi-weekly basis.
- Provide guidance to clients regarding vocational, educational, family, social, and overall daily life choices.
- Active participation in linking clients to resources.
- Assist with other daily Organization responsibilities as needed.
Education:
High School Diploma; B.A /B.S. Preferred, experience or education related to understanding mental illness and working with clients diagnosed with a mental illness strong interpersonal communication, and client-centered communication skills.
Preferred Skills:
Understanding of general psychosocial standards and medical practices associated with the treatment of mental illness; Bilingual; Work or volunteer experience in a mental health setting.
